Acknowledgment of risk, waiver and release of liability

I understand that karate is a physical, contact activity that includes kicking, striking, blocking, falling and partner drills, and that participation carries a risk of injury. I voluntarily permit my child to participate in the Highlands Kenpo Karate class held at the Highlands Recreation Center. I agree to release instructor Rick Yinger, the Town of Highlands, and the Highlands Parks and Recreation Department from all liability in case of an accident or injury during the duration of the class, and I agree to indemnify and hold harmless the same. I certify that my child is physically able to participate and that I have adequate insurance to cover any injury or damage that may be suffered while participating, or else I agree to bear the costs of such injury or damage myself. In an emergency, if I cannot be reached or am unable to consent, I authorize the instructor to obtain emergency medical treatment for my child.

Permission to be photographed and/or videotaped

I give permission for my child to be photographed and/or videotaped during the Highlands Kenpo Karate class at the Highlands Recreation Center. I understand that my child's photo or video image may be used in media articles, flyers, social media, presentation materials, or program materials.
